This service offers structured, in-home support to individuals with developmental disabilities who reside with family or in their own residence. It includes personal assistance, skill development, supervision, and behavioral support to promote increased independence and daily functioning. Services are provided in alignment with the Individual Support Plan (ISP) and tailored to each person’s needs and goals. Our team also collaborates closely with families and support coordinators to ensure consistency and effective care delivery.
Our services include but are not limited to:
1. Personal Assistance Services (as defined in Virginia’s DD Waiver Manual)

Support with Activities of Daily Living (ADLs):
Bathing
Dressing
Grooming
Eating
Toileting
Mobility
Transfers - Medication administration (per DMAS regulations and within provider scope)
- Monitoring of health status and basic health-related tasks (e.g., vital signs, reminders)
- Meal preparation and feeding assistance aligned with dietary requirements
- Household tasks necessary for health and cleanliness (e.g., laundry, cleaning, organizing)
2. Skill Building and Training (per Individual Support Plan – ISP)
- Self-help training (e.g., dressing independently, toileting)
- Money management (e.g., budgeting, purchases)
- Support with routines, task organization, and time awareness
- Travel and mobility skills (e.g., community navigation, public transportation)
- Personal safety education (e.g., emergency response, fire safety)
3. Behavioral Support Implementation
- Carrying out Positive Behavior Support Plans
- Tracking and reporting behavioral data to specialists or case managers
- Crisis de-escalation (with appropriately trained staff)
- Encouragement of positive social and behavioral habits
4. Socialization and Community Engagement (as authorized)
- Communication support, including AAC device use
- Participation in community activities: recreation, religious/cultural events, volunteering
- Support in building peer relationships
- Assistance with educational or employment-related goals (if included in ISP)
5. Health Maintenance Activities (per DMAS guidance)
- Follow-up support with healthcare tasks (e.g., scheduling appointments)
- Accompaniment to therapy or medical visits (if included in ISP)
- Assistance with durable medical equipment and assistive technology use
6. Family/Caregiver Training and Support
- Caregiver instruction based on ISP strategies
- Respite care (if authorized in ISP)
